At Cardiology, we handle all cardiovascular diseases from cardiovascular emergency 24 hours a day. The doctors at Cardiology are enthusiastic doctors who have gained a lot of experience and gathered in our cardiology department from all over the country. The medical equipment is fully equipped with leading-edge equipment, especially the cardiac catheterization equipment is equipped with the latest angiography equipment, and the hybrid operating room necessary for next-generation catheter treatment is also equipped. A team specializing in catheter treatment handles catheter examination and treatment.

 

Coronary catheterization

We perform more than 1,000 cases of coronary catheterization annually, which is one of the top-class experienced cases in Japan. Furthermore, even for urgent treatments such as acute myocardial infarction and unstable angina, a system in which a cardiologist is always on standby in the hospital provides the highest level of treatment that does not change 24 hours a day. For the purpose of reducing the burden on patients, we also perform one-day cardiac catheterization and treatment according to the degree of illness.

Peripheral vascular catheter treatment

In our department, we open a foot care outpatient clinic once a week (Monday afternoon) for peripheral vascular disease associated with the increase in diabetes and dialysis patients, and diagnose and treat arteriosclerosis. We have a vascular laboratory equipped with ABI, skin perfusion pressure test, peripheral vascular ultrasonography, CT, MRA and other vascular screening tests for quick diagnosis. If necessary, we mainly perform less invasive peripheral vascular intervention treatment (catheter treatment), and in addition to lower limb artery intervention treatment, we also perform renal artery and carotid artery intervention treatment. The number of cases of peripheral vascular intervention treatment exceeds 400 every year, which is one of the best in Japan. We also provide carotid artery stent treatment for patients with cerebral infarction and transient ischemic attack, which has been approved since 2008, in collaboration with the Cranial Nerve Center.

Valvular disease catheter treatment

In our department, in Surgery, we set up a heart team in the hospital and percutaneously for percutaneous aortic valve implantation (TAVI) for aortic valve stenosis and mitral regurgitation associated with severe heart failure. We perform mitral valve clip technique (MitraClip). For patients at high risk of heart surgery, we have the option of treating minimally invasive valvular disease with a catheter.

Rehabilitation is also substantial

Even athletes need rehabilitation after surgery on their legs and arms. Similarly, in the case of heart disease, cardiac rehabilitation is required for rehabilitation. Cardiac rehabilitation is performed for the purpose of returning a patient with heart disease to social life as soon as possible and preventing the recurrence of heart disease. We have set up a specialized cardiac rehabilitation center, and during the recovery period from heart disease, the cardiac rehabilitation team provides multifaceted guidance on exercise therapy, dietary guidance, and drug guidance to care for patients' daily lives.

We operate a mobile CCU

An ambulance for cardiologists dedicated to our hospital. If requested by a nearby medical facility, a doctor will be dispatched in an ambulance equipped with a respirator, defibrillator, auxiliary circulatory device, etc. Echocardiography is also possible, aiming to start initial medical care from the field.

[Event Report] 6th Yokohama Live Demonstration

The 6th Yokohama Live Demonstration was held on April 19th and 20th, 2024.

 

 

On April 19th, we performed a live EVT, and on April 20th, we performed Coronary PCI using Hybrid all day.

 

 

A total of six CTO lesions were treated using EVT, with three treated by our hospital's Mori, and all were successful without any problems.

 

 

PCI was also performed to treat five cases of CTO, two of which were treated by Ito and two by Kobayashi. Although there were some difficult cases, all were successfully treated without any problems.

 

 

This time it was a hybrid event, with people attending the Grand Hall in Shinagawa and also viewing the live stream online from across the country.

 

 

This online live event started during the COVID-19 period, and it has become possible for teachers who normally cannot meet in person to participate, and this time we were able to have many teachers from rural areas take part.

 

 

The drawback of holding a hybrid event is the cost, which is a big burden for the organizers, but we have seen that there is a lot of demand for it, so we will continue to make efforts to continue it in the future.

 

 

For this PCI live event, we treated very difficult cases that are not usually performed live, but as a result, we were able to discuss many issues and we believe that it was a very meaningful event for the many doctors watching.

 

 

We would like to express our sincere gratitude to all the staff involved in our hospital, the various companies that supported us, and the administrative staff.

The next live event will be held at our hospital in July, TOPIC2024.

Information on the 6th Yokohama Live Demonstration

The Yokohama Live Demonstration, the largest event Cardiology at our hospital, will be held from Friday, April 19th to Saturday, April 20th, 2024.

This year, the event will be held as a hybrid event, but we ask that you participate on-site as a general rule. The event will be held at the original Shinagawa Grand Hall.

EVT will have a live show on April 19th and PCI will have a live show on April 20th.

This sixth live event will be held at Tobu Hospital, and many doctors have described it as a highly challenging event, as it often deals with highly complex cases that are unique to the hospital.

This year's guest operators for EVT will be Dr. Kazunori Horie (Sendai Kosei Hospital), Dr. Naoki Hayakawa (Asahi Central Hospital), and Dr. Takanori Hirano (Toyohashi Heart Center), and the guest operator for PCI will be Dr. Junji Yajima (Cardiovascular Research Institute).

Information on EAST live 2024

EAST live 2024, which has been held every year recently, will be held online on Friday, February 16th, 2024.

This year, in keeping with the recent concept of reducing live event costs, the event will be held online only.

This live show is held online, and is one of our most popular, with people from all over the country participating every year.

We are currently selecting cases, and will likely be hosting only very complex PCI cases, which will be different from our usual live shows.

Currently, we have at least DCA (LAD and Cx) and CTO cases scheduled.

We will also be talking about Tip detection during the luncheon.

[Event Report] Jcalc2024 held

Jcalc2024 was held at the Tokyo Conference Center Shinagawa on January 19-20, 2024, and Yoshiaki Ito, Director of our hospital's Cardiovascular Center, served as the chairman.

In addition to lectures and hands-on sessions, the event also featured a live broadcast of PCI from our hospital on both the 19th and 20th.

 

 

 

A total of six patients with angina pectoris accompanied by calcification were treated over the two days. The surgeons were all directors of Jcalc, and included Dr. Shigeru Nakamura from Kyoto Katsura Hospital, Dr. Takashi Ashikaga from Musashino Red Cross Hospital, Dr. Gotoku Shibata from Miyazaki City and County Medical Association Hospital, Dr. Manabu Nakazawa from Kinki University, and Yoshiaki Ito from our hospital.

 

 

 

 

It appears that almost all of the devices for calcification currently available in Japan were used live, and although all cases were complex, treatment was successful in all cases without any major issues.

 

 

 

Many other experts also attended the lectures and other events, which I believe resulted in very meaningful discussions.

 

 

 

Next year's jcalc2025 will be held in Osaka, so we look forward to your cooperation and participation.

Information on Jcalc2024

A research seminar called J-calc 2024 will be held at the Tokyo Conference Center Shinagawa from January 19th to 20th, 2024.

The chairman of this meeting will be Yoshiaki Ito Saiseikai Yokohamashi Tobu Hospital, and the meeting will be held over two days with two pillars: lectures and live demonstrations. The live demonstrations will be live-streamed from our hospital, and we plan to do about six live demonstrations over the two days.

This year marks the third time this conference has been held, and as its name suggests, it specializes in the treatment of calcified lesions of the coronary arteries. Calcified lesions are also the most difficult hurdle we must overcome in coronary artery treatment.

However, there are still many unknowns regarding how we should use the knowledge, techniques, and tools we currently have to achieve successful treatment.

We hope that this conference will have you consider the main theme of how we should tackle calcification lesions at present. We have also invited many experts in this field from all over the country, and hope that you will take this opportunity to get up to date on the latest information on calcification lesion treatment.

Information on CTO summit VII held on December 1st

On Friday, December 1st, 2023, we will be holding CTO Summit VII, the last major event of the year Cardiology.

This year's event will also be held in a hybrid format, with both in-person and nationwide streaming.

This year's guest operator will be Dr. Atsunori Okamura from Sakurabashi Watanabe Hospital, as in last year.

Kobayashi and Ito from our hospital will perform the procedure.

The event will be held throughout the day, and we plan to treat 5 to 6 CTO cases.

[Event Report] SAMURAI style DCA live course 2023

On October 27, 2023, we held a live concert called SAMURAI style DCA live.

 

 

This live performance of catheter intervention treatment is probably the only one held at our hospital in Japan, and we held it again this year, following last year.

 

 

The live event was held entirely online, so it was watched by many doctors and medical staff from all over the country, from the north to the south.

 

 

The live event was divided into three parts, and the treatment of three cases was live-streamed.

 

 

In the first case, Dr. Tsutsumi performed DCA on the diffuse stenosis just proximal to the LAD, created landing, and then placed a DES only in the diffuse lesion.

 

 

In the second case, Dr. Kobayashi performed DCA for just proximal focal stenosis of the LAD. This lesion was focal stenosis, so the plaque was completely debulked and the treatment was completed with DCB without inserting a stent.

 

 

In the third case, Ito treated an ostioma lesion in the Cx. In this case, there was severe stenosis with calcification from the LMT to the LAD, and based on the results of IVUS, Diamondback was performed on that area first, and then DCA was performed on the Cx. As a result, a lumen was obtained in the Cx with DCA alone, and treatment was completed with DCA alone.

 

 

DCA is not yet a widely used device, and the procedure is complicated, so many doctors are hesitant to use it. However, we believe that you cannot become a top-class operator without mastering the DCA procedure. We would like you to learn it in order to avoid careless LMT stent placement.

Currently, in addition to percutaneous coronary intervention (PCI) and peripheral vascular intervention (EVT), catheter intervention for structural heart disease (SHD) is being actively performed in Japan. Among SHD procedures, percutaneous aortic valve implantation (TAVI) for aortic stenosis (AS), percutaneous mitral valve clip (MitraClip) for mitral regurgitation, and percutaneous left atrial appendage closure system (WATCHMAN) for the prevention of cerebral infarction are unavoidable paths for cardiovascular interventional physicians in the future.

Our hospital started TAVI in February 2014, and as of September 2023, we have performed 553 cases, MitraClip in November 2018 (96 cases), and WATCHMAN in August 2021 (36 cases). Our hospital has no barriers with cardiac surgery, and we decide on treatment methods by exchanging frank opinions at heart team conferences. In addition, we are one of the leading PCI and peripheral vascular EVT facilities in Japan, and we are one of the TAVR specialty facilities (38 facilities nationwide, as of August 2021) by the Japanese Society of Transcatheter Valve Therapy (JTVT). On the other hand, we also conduct multi-center collaborative research such as the OCEAN registry, and have disseminated a lot of evidence to date.

SAMURAI style DCA live course2023 will be held on October 27th, 2023.

SAMURAI live information

On September 22, 2023, we will be holding our first SAMURAI live in a year.

This live will be a completely web-based event that will include all complex lesions of coronary intervention.

It just so happens that the AI-OCT Ultreon version 2 has been updated, and we plan to perform OCT guided PCI for open vessels. Ultreon ver.2 is an advanced version of AI OCT, and although it is not yet widespread in facilities, we are pleased to be able to introduce any highly advanced functions that we are very excited about.

The 3D function is also now available, making it much easier to use and view than previous 3D displays.

We are also planning to treat several CTO cases, and are preparing to provide a complex PCI live event all day long.

Our hospital held a live demonstration of open vessel complex PCI on July 13th, and a live demonstration of CTO cases only on July 15th.

 

 

 

With the cooperation of all our staff, we were able to secure a total of 13 cases and hold the event without any problems, so we are relieved that we were able to do so without any problems. At the same time, we would like to express our sincere gratitude to all those involved who helped us.

 

 

 

The Complex open vessel PCI live event held on July 13th was the first of its kind for TOPIC, and we took great care in selecting cases and surgeons for the event.

 

 

 

On the day, a total of six cases were treated by Dr. Yoshifumi Kashima of the Sapporo Cardiovascular Clinic, Dr. Naoe Isomura of Showa University Northern Hospital, Dr. Shinya Okazaki of Juntendo University, Dr. Fumitoshi Hosaka of Okamura Memorial Hospital, and Dr. Masakazu Tsutsumi of Saiseikai Yokohamashi Tobu Hospital (our hospital).

 

 

 

This is my personal opinion, but I think that the calcified lesions were quite difficult and the surgeons had a hard time treating them. In addition, in the two cases where DCA was performed, the targeted debulking was successful, in one case a stent was placed after making a landing for the stent, and in the other case treatment was completed with DCA alone. I think this was the first time that DCA was properly performed at TOPIC.

 

 

 

We believe that each of the live cases provided a very fruitful opportunity for discussion.

 

 

 

Ultimately, all cases, including preliminary cases, were successfully treated without any complications.

 

 

 

On July 15th, we held a live performance featuring only CTO cases.

 

 

 

A total of five doctors, including Dr. Yasushi Asakura of Aqua Rehabilitation Hospital, who is the representative organizer of TOPIOC, Dr. Takayuki Ogawa of the Jikei University School of Medicine, and Dr. Ito from our hospital, as well as guests Dr. Yuji Oikawa of the Cardiovascular Research Institute and Dr. Masaki Fujita of Edogawa Hospital, attempted to treat seven cases of CTO.

 

 

 

Complex CTO treatment was performed using various techniques, including antegrade wiring, kissing wiring (after retrograde wiring), and IVUS-guided tip detection intraplaque trucking, and the CTO lesion was successfully patented and reperfused in all patients.

 

 

 

In a high-risk case treated by Ito, in which the entire procedure was not completed, the CTO was successfully passed using antegrade GWE, and the LAD just proximal was debulked using DCA, avoiding cross-over stents from LMT and placing a stent only in the occluded CTO area, completing the treatment.

 

 

 

More than 1,000 people attended the event, and I understand that the event was a great success. I feel a sense of relief that the two major events held at our hospital in the first half of the year (the first being the Yokohama Live Demonstration) have come to an end.

 

 

 

The second half of the year is packed with live events, starting with the CVIT live, followed by the SAMURAI live demonstration, DCA live demonstration, CTO summit, and Jcalc2024.

[Event Report] 5th Yokohama Live Demonstration

The 5th Yokohama Live Demonstration, the largest event in the first half of the year for the cardiovascular department at our hospital, was held from April 21st to 22nd.

 

 

 

Peripheral vascular treatment was conducted on April 21st, and coronary intervention treatment was conducted on April 22nd, both of which were conducted throughout the day. More than 350 medical professionals from around the country participated and watched the sessions online.

 

 

 

In peripheral vascular treatment, six cases of chronic total occlusion were successfully treated, including preliminary cases, and in coronary artery intervention, a total of five cases of chronic total occlusion were successfully treated, allowing us to conclude the meeting.

 

 

 

The coronary artery intervention cases included several very challenging lesions, in which we had to use a retrograde approach via superficial blood vessels or the IVUS guide tip detection method. Although these were difficult, we were able to complete the treatment without any complications in all cases.

 

 

 

This year, we have many live events planned, including TOPIC2023.

[Event Report] CTO summit VI

The 6th (6th year) CTO summit VI was held on December 2nd, 2022.

 

 

This year, we treated a total of 6 cases, including a very tough CTO case.

 

 

Two cases were treated by Ito, three by Dr. Kobayashi of our hospital, and one by Dr. Okamura of Sakurabashi Watanabe Hospital, our guest operator.

 

 

In addition to the Antegrade approach and Retrograde approach, this time Tip detection ADR was actually implemented in preliminary cases, and I believe that we were able to hold the latest CTO treatment live.

 

 

I would like to express my heartfelt gratitude to many professors from all over the country who watched and attended the conference, as well as the many professors who came to the MIC venue, which allowed us to proceed with the lively discussion.

 

 

We held 6 big live concerts this year, which is the largest number of events ever.

 

 

We have already received an offer to hold more live shows next year than this year, and we would like to tighten our hearts together next year as well.

[Event Report] Rotablator Online Preceptor LIVE

Rotablator Online Preceptor LIVE was held at our hospital (Saiseikai Yokohamashi Tobu Hospital) on the afternoon of October 21st. An educational live demonstration course for teachers who have newly obtained facility certification and are beginning to use the Rotablator.

 

In the first case, Yoshiaki Ito, the director of the center, acted as the operator, and it was a case with a severe calcified lesion in the proximal part of the LAD and bending of the LAD. Debulking was performed on a regular basis, resulting in an effective vessel preparation.

 

In the second case, Tsutsumi acted as the operator and performed Rotablation for the LAD mid calcified lesion. Since the lesion length itself is short, it may have been possible to dilate using a cutting balloon, etc., but if ballooning was performed without debulking, the healthy side would be overdilated and there was a risk of perforation, so balloon dilation can be safely performed. It was Rota for lesionmodification. As a result, I think it was very well modified and effectively prepared.

 

In the third case, Dr. Yoshiaki Ito, the director of the center, performed PCI again for a highly calcified lesion of RCA. Since it was a large vessel, the initial burr size started at 2.15 mm, which may have been a thrilling PCI for a beginner teacher, but this is also a PCI that considers guidewire bias, vessel size, calcium distribution and thickness in imaging. Since it was all sizing, I think I was able to show my skillful judgment and skill. All cases were successful without complications.

 

Tsutsumi gave the lecture and talked about the basics of Rota such as adaptation of the Rotabalator, determination of burr size, wirebias and short-cut, but the case was progressing smoothly, so I had a little time to spare. , As Tips & Tricks, we also talked about indications and tricks for Rota combined with Guiding extension catheter.

 

It was an educational live performance and lecture for teachers at facilities that are new to Rota and those who are still inexperienced. I would appreciate it if you had a good experience that will be useful for your PCI in the future.

 

Complex cases also gather from nearby areas at our hospital, and there are many Rota cases, and many experienced doctors are enrolled. There are many techniques and tips & tricks unique to our hospital, and in the past, we had the opportunity to see and learn from teachers outside the hospital at workshops. Due to the COVID epidemic, we are not able to hold the Rota workshop at the moment, but we look forward to your participation when the day comes when we can revive it.

 

 

 

[Event Report] SAMURAI Style DCA Live Course Course

The SAMURAI Style DCA Live Course was held on September 30th.

 

Thank you very much for participating in the live broadcast from our hospital, which was held in the form of web distribution, and many people from all over the country participated in spite of the busy time of daytime on weekdays.

 

This live is the final of a total of three live courses, and the first was a video live in March this year, focusing on DCA for LAD ostial lesions, and Tsutsumi acted as the operator. For the second time, Dr. Kobayashi of our hospital acted as an operator and watched DCA for LCx ostial lesions in a live video in June.

 

Since DCA was revived in 2016, this is the first attempt in Japan to offer a live course that professionally demonstrates DCA procedures. Three surgeons in the hospital performed the treatment.

 

We planned to treat 3 cases, but the treatment progressed smoothly and we had time to spare, so we showed everyone 4 cases including preliminary cases.

 

In the first case, Tsutsumi acted as the operator, resected the lesion at the entrance of the LAD with DCA, and when the % PA was reduced to about 50%, DCB was used for finishing and stentless treatment was performed.

 

In the second case, Dr. Kobayashi acted as the operator, and the lesion at the entrance to the LAD was also stentless treated with DCA + DCB. Because the treatment of the second case was completed earlier than planned, Dr. Yoshiaki Ito gave a lecture on DCA in a hurry. I did. In the fourth case, Dr. Kobayashi acted as the operator again and performed DCA for a highly difficult diffuse lesion of LAD.

 

All procedures were completed without complications.

 

For the doctors who are already working on DCA, I think that through this live, they were able to reconfirm the DCA method and technique tips, and brush-up their understanding and interpretation of IVUS. I am proud that I was able to deliver the Tips & Tricks of.

I think that many doctors who are about to start DCA and those who are interested but have never seen DCA think that DCA is a treatment with a high threshold. Looking at this live, the impression that everyone had expected was that "DCA can be performed in a short time and safely", and "It is not difficult to synchronize angiography and IVUS if you understand the basics". I think it will be a great success if the image changes.

Our hospital will continue to actively work on DCA, and we would like to disseminate data such as mid- to long-term clinical results of DCA and Tips & Tricks through academic conferences and live performances.

12/2 CTO summit VI

On December 2nd, we will hold the annual CTO summit again this year.

This live is a live demonstration of only CTO cases all day long alongside the Yokohama live,
We are planning to hold a live center again this year. This year we are planning to hold the event completely online.

This year's guest operator will be Dr. Atsunori Okamura from Sakurabashi Watanabe Hospital, and Kobayashi and Ito from our hospital will perform the procedure.

For the lecture during the luncheon time, we are asking Mr. Okamura to give a lecture on 3D wiring, etc.

The latest CTO treatment trend, angio-based 3D wiring, IVUS-guided 3D wire, and tip detection method to tip detection ADR, etc. We are looking forward to holding the event, so we hope that everyone involved will be able to participate.

Held on 9/30 SAMURAI style DCA Live Course

On September 30th, we will hold a live demonstration of only DCA called SAMURAI style DCA Live Course.

The number of doctors and facilities that can use DCA is limited, and there are not many indications for DCA, so it is considered a rare technique that is rarely seen in live demonstrations, let alone in regular clinical practice.

At our hospital, at least five doctors, including Dr. Ito, Dr. Yamawaki, Dr. Kobayashi, Dr. Mohri, and Dr. Tsutsumi, are well versed in the treatment of DCA.

DCA has not yet been performed but wants to try it, has been performed but has not been able to experience many cases and is struggling to improve, wants to step up further, etc. I was thinking that I could hold a live that could be done.

So far, we have held video live twice on the web, but this time we decided to actually hold at least three cases of DCA treatment live.

It is probably the first time in Japan that a live demonstration of only DCA will be held. We would like you to experience the DCA treatment that we usually perform while discussing what you want to know and hear in the actual treatment.

[Event report] Live report with Taiwan

A live demonstration called Taiwan & Japan CTO cases Virtual Forum was held on 8/23 the other day.

 

 

This live show was offered by a Taiwanese person to our hospital, and we performed a CTO PCI live demonstration from our hospital. The first case in the morning was treated by Dr. Ito. In this case, obstruction from just proximal of the LAD and calcification were also observed, so a retrograde approach was performed, and after passage through the channel, retrowire crossing was successfully performed under IVUS guidance, and stent placement was finally successful.

 

 

In the second case in the morning, Dr. Kobayashi performed the treatment, proceeded with the procedure with an antegrade approach to the CTO of RCA, and succeeded in passing the wire to the distal true lumen.

 

 

In the afternoon, there was a live broadcast from Taiwan, and the treatment of two CTO cases was completed while doctors from both countries discussed Taiwanese CTO cases.

 

 

There were more than 1,500 viewers on the day of the event, and I think it was a very meaningful time to be able to exchange opinions on both sides of the web live demonstration that transcended national borders, even in the corona whirlpool.

 

 

Our clinic was closed on Saturdays, but we were able to hold live performances thanks to the cooperation of many people involved, such as web connection tests and case recordings.

TOPIC2022 was held completely online from July 7th to 9th, 2022.

6/3 DCA Advance Video Live

Coming 6/ 3, the DCA video live will be held as the second series.
Continuing from the first lecture, this time we will focus on the DCA of the circumflex I will give you an explanation.

There are various techniques for performing DCA of the circumflex, You have to enforce it on the acquisition of knowledge.
In this live performance, I learned techniques and I that are not written in the textbooks necessary for the implementation. I would like to explain in detail the points of VUS interpretation.

The 4th Yokohama Live Demonstration was held at our clinic on April 22-23, 2022.

  

 

We were able to hold Hybrid in the midst of the corona wreck, and many people from all over the country watched and attended, and we were able to successfully complete the meeting. We would like to express our heartfelt gratitude to all those who participated.

  

 

This live is the one that we put the most effort into in our clinic, and we are proud that we were able to hold a very satisfying live this time as well.

  

 

As for EVT, I was able to see next-generation EVT in Dr. Hirano's AI US guide EVT, and Mohri at our hospital treated a total of three CTO cases brilliantly.

  

 

PCI Live performed a total of 6 cases of CTO and DCA treatment mainly at our hospital Kobayashi.

  

 

Kobayashi's case was able to demonstrate various forms of wiring of the IVUS guide, and I think that I was able to learn the latest treatment of tip detection live.

  

 

Ito's case is also based on Angio, but I think that 3D wiring was performed and tough CTO treatment was successful, and various information could be provided.

SAMURAI DCA Video live will be held from 19:00 on March 18th, led by our doctor.

This video live is aimed at improving the skills of doctors who have started DCA but feel that it is difficult to treat with confidence without a proctor. It is scheduled to be held in a series of 3 times, and the 3rd time is scheduled to be live.

In this first installment, we will focus on the ostium of the LAD, which is the most shocking treatment site in DCA, and Dr. Tsutsumi of our hospital will introduce the procedure.

I would like to unravel the ABCs of DCA through case studies. I would like to thoroughly delve into the points so that I can understand everything from the interpretation of IVUS to the tips when implementing DCA with commentators.

EAST Live was held

2022.2.25 EAST live was held for the second time this year.
Due to the corona vortex, we have delivered a total of 5 complex lesion treatments on the web this year as well.

  

 

In the first case, Dr. Tsutsumi performed DCA of the LAD and finally succeeded in stent placement avoiding crossover.

In the second case, Dr. Yamawaki treated a ture bifurcation lesion of LMT using a stent after performing a rotablator.

  

 

In the third case, Ito treated the LAD proximal lesion by debulking only with DCA without using a stent.

In the 4th case, Dr. Kobayashi treated the RCA CTO using IVUS, and in the 5th case, Ito passed the LAD CTO with Antegrade and the treatment was successful.

  

 

This time, due to the influence of Corona, there was no guest operator, and the live event was held by four operators in our hospital.
Many teachers from all over the country watched it, and I think that we were able to hold a very educational live performance.

First of all, we would like to inform you about the Japan Calcification club (J-Calc: Japan Calcification Club (J-CalC)) that will be held from 2022.1.28 to 29 this month.

This is the first live and academic event specializing in calcified lesions.

From our hospital all day on 1.28, we plan to have a PCI live on the Web nationwide on Hybrid.

I don't think there's ever been a live like this before, and it's got a lot of content, so if you're interested, I hope you'll join us.

[Event Report] CTO summit V was held

On November 26, 2021, the CTO summit V hosted by Medtronic was held at our clinic and ended successfully. The corona situation has not completely subsided, but this time it was held in a hybrid format using both the web and Medtronic's conference room. I would like to express my sincere gratitude to the chairperson, the commentators, the teachers who gave presentations, the viewers, and the people of Medtronic who cooperated with us.

  

 

  

 

  

 

  

 

Initially, we planned 6 cases of CTO and 1 case of DCA, but the 2 cases in the morning took a long time. Only 3 cases of CTO could be performed. The results were quite difficult, but we succeeded in completing all 3 cases while preserving the bifurcation (pre and post images of 3 cases have been uploaded).

 

Until now, we have performed various live performances at our clinic, but looking back, I don't think I have ever had such a difficult time. Once again, I am keenly aware of the difficulty in picking up cases suitable for live demonstrations, but I think that the discussion was rather lively with a wide variety of comments from the teachers during the live demonstration.

 

We will continue to hold live demonstrations at our clinic, and we would like to ask for your continued cooperation. Thank you very much for this time.

[Event report] SAMURAI LIVE was held

On November 5, 2021, we held an intervention live called SAMURAI LIVE. We have held many live demonstrations every year, but this time we did our first live distribution overseas. Of course, we held the live in English, and we are proud that we were able to hold a very educational live without any communication problems.

 

The surgeons were Tsutsumi, Kobayashi, Yamawaki, and Ito, who treated a total of 6 patients, all of whom were successfully treated.

 

I also realized that using the web makes it possible to do live distribution overseas without spending any money.

 

At this live show, I realized once again that Japan is one or two steps ahead of the rest of the world when it comes to treating complex lesions, including chronic total occlusion. I was convinced.

 

We will continue to devote ourselves to study and to perform interventional treatment based on patient first. And if there is an offer for such an event in the future, I will definitely work on it positively.

 

I would like to sincerely thank all the staff who were involved in the preparation and the staff who were involved on the day. Good teamwork is one of the strengths of our catheterization team, so I am always reassured.

[Event Report] TOPIC2021 has ended

TOPIC2021, a big event in the first half of this year, was held from July 9th to 10th and ended.

  

 

It was a great success with many viewers from all over the country.

Last year, we postponed the event, and this time we held a complete web-format live for the first time in TOPIC, but I am relieved that we were able to hold it without any major problems.

  

 

On July 10th, we performed a live demonstration from our hospital all day long and treated 6 cases of chronic total occlusion. Although there were some minor problems, we were able to complete the treatment in all cases within the allotted time, and we were able to hold a satisfactory live demonstration.

The 3rd Yokohama Live Demonstration was held on April 16-17, 2021.

This time, we held a hybrid event in the midst of the corona vortex and performed a face-to-face live.

  

 

Thank you very much for coming to the venue for some of the teachers in the Kanto area. I received words of gratitude from various teachers for holding a face-to-face live.

  

 

On April 16th, EVT live was held all day and all CTOs were treated. A guest doctor was also welcomed and all cases were treated successfully.

  

 

On April 17th, we performed PCI live all day and performed 6 CTOs and 1 DCA.

  

 

Two of the six CTOs also performed DCA, making a total of three DCAs.

  

 

There were some cases with a fairly high degree of difficulty, but we were able to finish the treatment without any problems in all cases.

  

 

This live viewing was probably attended by people from all prefectures, so it was a live show where you could really feel the benefits of web live.

  

 

And I received a message from a teacher who actually lives in a rural area, saying that it would be helpful to use the web. That would have been unthinkable a few years ago.

  

 

It was a great harvest for us that hospitals and doctors we did not know at all watched it.

On February 25th, we held a live performance called Rotablator Online Precepter Live at Closed (facility only).

 

 

Half a day in the afternoon, Ito treated Rotablator with explanations for two cases. One case was a normal case, but the second case was a very complex lesion that was treated with Rota from LMT to LAD and Cx after the live, but the treatment was completed without any problem.

 

 

We usually hold live performances centered on many CTOs, but this time it is a live performance specializing in Rotablator for calcified lesions that also serves as a course, and it takes time to examine cases in advance and to have lectures in the middle. Was held.

 

 

It was a Preceptor Live that I could hold while having two-way discussions with teachers from many hospitals from Aomori in the north to Okinawa in the south, and felt the advantages unique to Online live. Perhaps the other teachers feel the advantage of being able to communicate with distant teachers who are difficult to come to Tokyo or Kanagawa. EAST live was held on February 5, 2021

It was the first live of this year, but I am relieved to be able to hold it safely. The number of cases was reduced in this live, and the live broadcast was carried out with sufficient discussion and time.

  

 

In the first case, LAD Proximal lesion was performed by DCA alone, and in the second case, LAD calcified lesion was ablated by Rotablator under OFDI guidance.

  

 

And the third case was the CTO of RCA, but in fact, it was successfully treated with the Double CTO in combination with the Retorograde approach.

  

 

At the end, it was out of the scheduled time, but since it was held on the Web, we continued the live broadcast and was able to treat the CTO of RCA with Antegrade.

  

 

The end time was past 18:00, which greatly exceeded the scheduled time, but thanks to many staff and teachers who participated from outside, we are grateful to all the staff of this hospital for their success. The CTO summit IV was held on October 30th, the second time this year and the fourth time in total.

This CTO summit IV was an annual live performance for our hospital's cardiovascular system, which has been held once a year for four years, but although it was feared that it would be held this year, we are all relieved to hold it. .. A total of 5 CTO cases were broadcast live on Hybrid from morning to evening, and many people participated.

 

    

 

For this live broadcast, we distributed the video and audio on the Web using only the equipment of our own facility, but I think we were able to broadcast it all day without any major trouble. I think that future live performances will shift to this kind of event, and I realized the possibility.

2020/10/16 We held the first live​ ​web site at our hospital this year.

I was surprised that it was watched by nearly 300 people and was delivered to more people than usual face-to-face live performances.

On the day of the event, we had a great time with discussions by the chairman and the commentator, and we learned a lot.
In addition, I think that we were able to deliver a satisfying live stream, thanks to the cooperation of not only our doctors but also all of the comedics.
I also thought that the future of live performances could move in a different direction from the past, such as web distribution, and that it was a live performance that felt the possibility.

This is the first attempt, but I think it will be possible to hold a live performance that is unique to the Web and has a close sense of distance, so I hope that medical professionals will join us.

Other live performances are planned, so we will report them one by one.

2019.12.05 PCI treatment workshop was held.

The last PCI treatment workshop of the year was held on December 5, 2019.

Six teachers from all over the country came this time as well, and I think we were able to have lively discussions and tours from the case study meeting the day before.

In this workshop, in addition to 3 cases of chronic complete occlusion lesions, there were several cases of calcified lesions using rotablator and diamond bag, and I think that we were able to hold a workshop unique to our hospital that made full use of imaging modality. I will.

It was a busy year anyway, but I would like to do my best for the cardiovascular system so that I can carry out similar activities next year.

2019.11.14 A workshop on coronary intervention treatment was held.

A total of 11 Drs from all over the country participated this time as well, and meaningful and lively discussions were held starting from the case study meeting the day before the workshop.

A total of 11 cases of complex lesions such as chronic complete occlusion lesions, highly calcified lesions, and left main trunk bifurcation lesions were treated, and all cases were successfully treated.

Dr. Yamawaki, Dr. Kobayashi, Dr. Sakamoto, and Dr. Mouri treated typical chronic complete occlusion lesions by the Retrograde approach and highly calcified lesions that underwent rotablator diamond back. In addition, Dr. Yoshiaki Ito performed difficult chronic complete occlusion lesions that required ADR (antegrade dissection re-entry) technique and stentless treatment of the left main trunk bifurcation by DCA (directional coronary atherectomy). The number of cases was large, and the three catheter rooms were always in operation, and the participating teachers actively entered the catheter rooms. I think I was able to give a very good treatment while discussing with the teachers.

At this live, the guest operator teacher was not invited, and Cardiology Tobu Hospital acted as the main operator. A total of 10 cases were treated live, but all of them, such as chronic complete occlusion lesions, bifurcation lesions, highly calcified lesions, and stent restenosis lesions, are often encountered in daily clinical practice and often have problems with treatment policies. It is the real pleasure of the live demonstration to be able to discuss while looking at the actual procedure and solve questions in daily medical care. Very heated discussions were held this time as well, and the procedure was successful in all cases, which contributed to the patients.

 

Dr. Ito of our hospital treated 2​ ​cases of chronic complete occlusion (CTO) lesion and lesion of the left main trunk bifurcation. Both cases were introduced by other hospitals and were extremely difficult, but we were able to succeed successfully. The left main trunk lesion was treated by a treatment method called DCA, in which the coronary plaque was excised and without a stent. Since two types of antiplatelet agents must be taken after stent placement, there are concerns about bleeding events, especially in elderly patients. It takes skill to complete the procedure without inserting a stent at DCA, but from the data of this hospital, the results in the medium term are good and we hope to increase the number of cases in the future.

Three cases of chronic complete obstruction lesions were treated by Dr. Ito, Yamawaki, and Kobayashi, and ablation of calcified lesions using Rotablator and Diamondback 360 was treated by Dr. Yamawaki, Araki, and Kobayashi.

 

 

Chronic complete occlusion lesions were treated with a treatment device called Stingray, which is a new device that is not widely used in Japan, and all cases were successful.

The treatment was carried out all day, and there was a social gathering at night, so thank you for your hard work.

Dr. Yasunari Sakamoto played a central role in giving explanations while practicing OCT-guided interventions in actual cases. Through the cases, we had discussions with our Clinical Engineer, and trained a part of the OCT guide from Iroha of OCT.

           

 

The penetration rate of OCT is very low in other Asian countries, and most of the facilities are considering introducing it. At our hospital, which is energetically conducting OCT-guided interventions, we are actively accepting training and tours not only for domestic doctors but also for overseas doctors and medical professionals, and we would like to continue to practice them in the future. Peripheral vascular intervention will be performed by Dr. Miyamoto of Takatsu Central General Hospital, Dr. Ando of Kasukabe Central General Hospital, Dr. Miyashita of Nagano Red Cross Hospital, Dr. Tsuchiya of Kanazawa Medical University Hospital, Dr. Iida of Kansai Rosai Hospital, and Dr. Soga of Kokura Memorial Hospital. In each case, we received a highly difficult peripheral vascular treatment.

Tobu Hospital Dr. Hirano provided an ultrasound-guided treatment for superficial femoral artery occlusion lesions with explanations.

The coronary intervention was performed by Dr. Hosaka and Dr. Tarutani of Okamura Memorial Hospital, and Dr. Yajima and Dr. Oikawa of the Cardiovascular Institute as guest operators. I was treated.

Tobu Hospital Dr. Ito also treated chronic completely obstructive lesions.

The meeting ended with a hot message from Dr. Takanori Hirano and Dr. Yoshiaki Ito, the representative caretakers, who would like to develop this Yokohama live demonstration with a view to fostering mid-career-young doctors.
