Medical Document Counter information
Medical Document Counter is a reception desk where you can apply for the creation of any medical certificates for life insurance and disability pensions, application forms for sickness and injury benefits, individual forms for clinical investigations of specific diseases, etc.
Please note that in some cases, specified documents such as life insurance medical certificates may be required to be prepared by the patient.

What you need to apply
- hospital card
- Documents to be submitted
- Identification card (health insurance card, My Number card, driver's license, passport, etc.)
- Power of attorney (if someone other than the patient applies)
- A copy of the family register clearly indicating the relationship to the patient (if the application is made by a family member or relative of a deceased patient)
Document application procedure
① Fill out the necessary information on the "Medical Certificate and Other Document Application Form" available at Medical Document Counter.
Please fill in the special notes section of the application form with information such as the submission deadline and what you would like to communicate to the doctor.
② Please submit this document to Medical Document Counter together with the above “Items required at the time of application.”
Document Receipt Procedure
The documents will be handed over at a later date. It may take between two weeks and one month for them to be issued.
If there is a deadline for submission, please apply well in advance.
Once the documents are ready, we will contact you at the phone number you provided on your application.
However, if you receive your order by mail, you will not receive a phone call.
②Please prepare your hospital card and the ID of the person coming to collect the item, and speak to the counter.
③Please note that we may not be able to give you any documents that you have not received within six months after we have notified you that they are ready.

Notes
About "Life Insurance/Simplified Insurance Medical Certificate"
① Please apply after the day you are discharged from the hospital. In the case of surgery that involves a pathology test, please apply after your outpatient consultation after being discharged from the hospital. Other exceptions will be announced at the reception desk.
② Depending on the contents of the insurance contract, there may be cases where insurance premiums are not paid, so be sure to check the benefits details with the insurance company in advance (entries in the outpatient treatment column are subject to the condition that outpatient treatment is covered by benefits).
③An application is required for each clinical department (even if you are transferred to another department during your hospitalization, clinical department).
