For patients making an initial visit

As a foundation hospital of this region, our hospital provides specialized tests and examinations, emergency treatments, and hospitalized treatments to acute patients. Treatments of chronic diseases and everyday health care are given by neighboring medical institutions.

Reception hours for first-visit

8:30 a.m.–11:00 a.m. on weekdays (Monday to Friday)

  • If you visit our hospital or department for the first time, please come during these hours.
  • Some specialty outpatient departments have different reception hours. Please contact us for detailed information.

Closing days

Saturdays, Sundays, National holidays, and New Year Holidays
(December 29–January 3 in the following year)

  • Some departments have a non-consultation day.

Please bring

  • health insurance card (medical certificate and subsidy recipient certificate, if applicable)
  • referral letter from your family doctor (patient referral document)
  • identification card (with your photo on it)
  • Residence card
  • passport
  • Please make sure to bring a referral letter from your family doctor, because it tells us your medical information effectively.
  • Departments of Ophthalmology, Otorhinolaryngology, Oral and Maxillofacial Surgery, and Dermatology are for referral patients only. Please make sure to bring a referral letter with you, otherwise you cannot see the doctor. Other departments are basically for referral patients, too. So, please bring your referral letter. If you do not have a referral letter, you are asked to pay for a fee for treatment of patients’ choice at the first consultation in an amount of 5,500 yen.

Visiting to the Department of Obstetrics

Before visiting the Department of Obstetrics for the first consultation, please make sure that you make an appointment. Otherwise you cannot see a doctor.

Procedures to have your consultation

Fill out a Patient Registration Form available from the desk and submit it with your health insurance card, referral letter, and identification card to the first-visit reception counter at the General Reception. You can download the Patient Registration Form from here and fill it in advance to make your registration process go smoothly (Pre-registration is not available).

  • If you do not have a referral letter, fill out the Patient Registration Form and Medical Questionnaire and submit them to the reception counter.
  • If you already have your Patient ID Card, you don’t need to submit the Patient Registration Form.

    Complete “Patient Registration Form” and “Medical Questionnaire.”

    If you have your referral letter, you don’t need to fill out “Medical Questionnaire.”

    Submit “Patient Registration Form,” “referral letter” or “Medical Questionnaire,” and “identification document” to the first-visit reception counter at the General Reception.

    Issuance of your Patient ID Card and proceeding to the department or tests

    Submit your Outpatient acceptance receiptto the reception counter of the respective department or laboratory.

Patient ID Card

Patient ID Card is common in all departments. If you have your Patient ID Card, please make sure to bring it with you for your next visit.
If your Patient ID Card is broken or lost, it will be reissued with a charge of 220 yen.

Fee for special or specified medical care for your first consultation

If you do not have a referral letter (patient referral document) from another authorized insurance medical institution, you are asked to pay for a fee for treatment of patients’ choice in an amount of 5,500 yen (tax-included) separate from your medical fee for first consultation if:

  • you visit our hospital for regular checkups without instruction of follow-up visit from our hospital doctor;
  • you discontinued your treatment by yourself even if this visit is made for the same disease and symptoms as in your last visit; or
  • you make your visit for a disease for which recovery was once confirmed to the same department for the first time since then or to a different department for the first time.
  • Please note that you are asked to pay for the first visit fee to the Department of Oral and Maxillofacial Surgery (Dentistry) even if you paid the fee to another department.

For Follow-up visitors

If you have an appointment, please check in using the return visit reception machine on the 1st floor in the appointment reception time. Your health insurance certificate will be confirmed (once a month). Please note that you should bear the full amount of the bill if the certificate cannot be confirmed.
If you have an outpatient appointment at the Children’s Center for Health and Development or Mental Health Care Center, reception procedures will be performed without using the return visit reception machine. Even if you have an appointment on the day, please come to each department/examination room reception counter without going through the return visit reception machine.

Changing date and time of your appointment

  • Depending on a scheduled test or department, your request for appointment change may not be accepted.
  • You cannot change your appointment after the appointment day.

If you do not have an appointment:

Please come to the General Reception on the 1st floor between 8:30 a.m. and 11:00 a.m. (closed on Saturdays, Sundays, and National holidays). If you wish to make a follow-up visit to the “Department of Obstetrics, Gynecology, or Reproduction” even without an appointment, you can directly come to the reception counter of the Comprehensive Women’s Medicine Center (Block 26) on the 2nd floor.

  • If you have never visited the department before, your visit is regarded as a first visit.
  • Even if you have visited the department before and your treatment was finished, your visit is regarded as a first visit. In that case, if you don’t have your referral letter, you are asked to pay for a fee for special or specified medical care in an amount of 5,500 yen for your first consultation.

Fee for treatment of patients’ choice for follow-up visits

If you wish to visit our hospital for follow-up examination regardless of introduction and proposal for visiting another medical institution from our hospital, you will be asked to pay for a fee for treatment of patients’ choice in an amount of 2,750 yen separate from medical fee at the time of follow-up visit. The fee for special or specified medical care for your first consultation and the fee for treatment of patients’ choice for follow-up visits have been established by the Ministry of Health, Labour and Welfare to allow smooth functioning of hospitals and clinics.
